Abt 1835 - 1905

Samuel Archibald was believed to be about 70 years old when he passed from this life at Utah State Hospital. His death certificate states he had been a farmer, a former resident of Westwater, Utah (ghost town), marital status and family unknown. Samuel had been a patient for 12 days at the time he died.
Utah State Hospital Forgotten Patients Project: Samuel was among 600+ patients of the Utah Territorial Insane Asylum (now known as Utah State Hospital) between 1886 and 1960, buried in the Provo City Cemetery, most in unmarked graves, given a pauper's burial, alone without recognition or ceremony. The Utah State Hospital Forgotten Patients Cemetery Project requested donations to create a monument with individual plaques to honor each of these precious spirits. Thankfully, it was completed and dedicated to them October 10, 2018, in Provo City Cemetery.
Discrepancy: For death date between death certificate (7 Nov), cemetery record (7 Nov), USHFPP memorial plaque (1 Nov). Plaque is incorrect; doctor knew the date when signing DC.
